What to Bring to Your Appointment
To ensure your immigration medical exam goes smoothly, please bring the following items to your appointment:
- A valid government-issued photo ID (passport, driver’s license, or other official document)
- Form I-693, “Report of Medical Examination and Vaccination Record” (printed but not signed — you’ll sign it during your visit)
- Vaccination records (official records from your country or previous healthcare providers)
- List of current medications
- Eyeglasses or hearing aids, if applicable
- Payment for the exam (immigration physicals are not covered by insurance)
If you don’t have vaccination records, our clinic can provide blood tests (titers) or administer any missing vaccines during your visit.
Vaccination Requirements

What to Expect During Your Immigration Physical
Your immigration medical exam at Yakima Medical Clinic typically includes:

After the Exam
Once your examination and testing are complete, our Civil Surgeon will:
- Fill out and seal Form I-693 in an official USCIS envelope (do not open this envelope).
- Provide you with a copy of the completed form for your records.
- Submit any additional test results or vaccination documentation required for your application.
You can then submit your sealed envelope directly to USCIS with your immigration paperwork.
